There is an intramedullary lesion of central location within the conus medullaris at the level of T10-T12, displacing the nerve roots of the cauda equina. The lesion is composed of small cystic areas of low signal on T1 and T2, and a solid component of isosignal to the cord on T1, iso-to high signal on T2 with heterogeneous enhancement on postcontrast sequences. An area of vasogenic edema is noted above the lesion. Mild posterior vertebral scalloping at T11 with no bony destruction.
