Diagnosis almost certain

Presentation

Eyrthema nodosum for 3 months.

Patient Data

Age: 40 years
Gender: Male
x-ray

Marked bilateral hilar and left paratracheal lymphadenopathy, with infilling of the hilar angles and aortopulmonary window.

Lungs clear.

ct

Extensive mediastinal lymphadenopathy at multiple nodal stations, the largest 5.6cm in the subcarinal space.

Occasional lung nodule.  Nodularity of the fissures, most pronounced on the horizontal fissure.

Case Discussion

A textbook presentation of sarcoidosis with erythema nodosum and mediastinal lymphadenopathy on chest radiograph.

The CT findings further support the diagnosis with fissural nodularity.  At this early stage, no fibrosis is present.
