Presentation

Right homonymous hemianopia.

Patient Data

Age: 50 years
Gender: Male

Diffuse low attenuation in the left occipital lobe in the territory of the posterior cerebral artery.  

No hemorrhage.

T2, FLAIR high attenuation with corresponding avid diffusion restriction in the left occipital lobe.  Smaller focus of diffusion restriction in the left cerebellar hemisphere.

No hemosiderin deposition.

Normal posterior fossa.

Textbook presentation, CT and MRI appearances of an acute left posterior cerebral artery territory infarct.

Observe how intensely avid the diffusion restriction is in the infarcted occipital lobe served by the posterior cerebral artery.

An additional focal infarct is observed in the left PICA territory.
