Claw sign in Modic type 1 endplate changes

Hyperintensity is seen involving L4-5 intervertebral disc. Foci of intradiscal low signal compatible with gas
also seen. Subtle erosions are seen along L4 and L5 end plates with marrow edema. Minimal pre / left paravertebral edema seen.
Case Discussion
Findings suggest likelihood of type I Modic end plate changes. DWI shows the so called 'claw sign' which refers to a pattern of hyperintensity seen in degenerative changes in the spine. The presence of the claw sign makes spondylodiscitis less likely.
