The MRI sequences demonstrate a large sellar and suprasellar mass (5 x 2.7 x 2.5 cm) with compression and upwards displacement of the optic chiasma. It displays an iso-to low signal to the grey matter on T1, iso-to high signal on T2 with moderate enhancement on postcontrast sequences except for the areas of cystic change/necrosis seen in the suprasellar component. There is an extension to the sphenoid sinus through the destruction of the sellar floor/dorsum sellae with erosion of clivus.
Note enlarged adenoids confirmed as adenoidal hypertrophy on the ENT exam.
