Bilateral subdural hematomas

Diagnosis almost certain

Presentation

Patient with disorientation and inability to walk independently. History of head trauma and nasal bleeding two months ago.

Patient Data

Age: 65 years
Gender: Female
mri

Large, bilateral frontal and parietal convexity late subacute phase subdural hematomas with a few loculations secondary to membrane formation.

The largest axial dimensions of the subdural hematomas are 14 mm and 20 mm on the right and left sides respectively.

Balancing mass effect on the underlying cerebral hemispheres.

No transtentorial or foramen magnum herniation.

High signal foci in T2 and flair sequences at subcortical and periventricular white matter of both cerebral hemispheres depict microvascular ischemic events.

Case Discussion

There is a bilateral late subacute phase subdural hematoma.
