Cavernoma

Presentation

Seizures (episodes of blackout lasting for 30 minutes associated with nausea and headache. Around 6 attacks over the last 5 months.

Patient Data

Age: 20 years
Gender: Male

Left occipital subcortical well defined lesion showing characteristic popcorn" or "berry" appearance with a dark rim of signal loss due to hemosiderin. It elicits central hyperintense signal on T1 with an intermediate marginal bright signal on T2 and FLAIR with hypointense rim as well as peri-focal small gliotic changes of slightly bright T2 and FLAIR. It elicits signal loss and blooming on GE sequence and not restricted at DWI.

Case Discussion

MRI demonstrating a characteristic “popcorn” or "berry" appearance with a rim of signal loss due to hemosiderin, which demonstrates prominent blooming on susceptibility weighted sequences (T2*).
