Tuberculous arachnoiditis

Diagnosis almost certain

Presentation

Fever, backache for 3 months. Sudden onset paraparesis. Urinary incontinence followed by acute urinary retention. Past history of tuberculous meningitis 8 years back.

Age: 45 years
Gender: Female

There is leptomeningeal enhancement along the surface of the conus medullaris. Cauda equina nerve roots are clumped and adherent peripherally with the thecal sac (empty thecal sac sign).  At L5 level, theca and nerve roots are clumped together forming an enhancing soft tissue mass in the spinal canal. Note is made of reduced disc space at L3-L4 level with collapse of L4 vertebra secondary to tubercular spondylodiscitis.
