MR prostate: incidental iliac artery aneursyms

Diagnosis certain

Presentation

PSA 8.1, on Finasteride. DRE: hard prostate both sides.

Patient Data

Age: 70 years
Gender: Male

Prostate volume of 132mls.  PSA density 0.06ng/ml.

Essentially no peripheral zone remains.  Extensive BPH (PIRADS 2)

Normal seminal vesicles.  No pelvic lymphadenopathy.  No focal bone lesion.

3.7cm right and 2cm left internal iliac aneurysms.

Case Discussion

A number of incidental findings may be identified on prostate MRI.

These include;

a.  Sigmoid diverticular disease

b.  Colonic cancer

c.  Iliac artery aneurysms

d.  Hydroceles

e.  Bladder stones or transitional cell carcinoma

In this case bilateral internal iliac artery aneurysms are identified.
