There is caudal descent of the cerebellar tonsil through the foramen magnum.
An expansile intramedullary lesion is seen in the spinal cord extending from the cervical cord down to the conus medullaris (extends beyond the field of view). It appears hypointense on T1 and hyperintense on T2/STIR with areas of pulsation artefacts. This appearance is in keeping with a long segment syrinx.
