Seizures onset

Age: 50 years
Gender: Male
mri

There is no mass, hematoma, or demyelination.

Diffuse periventricular white matter heterogenous T2 hyper signal.

There is excessive basal ganglia calcification with T2 blackout on ADC followed by subcortical white matter and dentate nuclei T2* susceptibility artifacts.

Case Discussion

There are classic findings consistent with Fahr disease, not syndrome since its the primary manifestation at age of 50 years in the absence of hyperparathyroidism.
