Presentation
Cough and hemoptysis. The patient is on anticoagulant drugs.
Patient Data
Diffuse ground-glass alveolar opacification casting the pulmonary lobules in both lungs, predominately affecting both lower lobes. No interlobular septal thickening.
Incidental note of left parahilar homogeneous density lesion with smooth margins, splaying the left lower lobe bronchus.
Case Discussion
The presentation of hemoptysis with the CT features are in favor of diffuse alveolar hemorrhage (DAH). No interlobular septal thickening, suggestive of acute stage. The history of anticoagulant drugs aids in the diagnosis and excludes the possibility of Goodpasture syndrome.
The incidentally noted homogenous left parahilar lesion is suggestive of a bronchogenic cyst.
