Urinary bladder cancer

Diagnosis almost certain

Presentation

Gross hematuria and right flank pain.

Patient Data

Age: 85 years
Gender: Male
ct

A few non-enhanced simple cortical cysts are seen at both kidneys, with maximum diameters of 15 mm. Urinary collection system of right kidney is duplicated and moderate hydroureteronephrosis is present at right side.

Uncomplicated distal colonic diverticulosis is present.

A 26×18 mm exophytic soft tissue density mass with coarse calcified focus is present at proximal of gastric lesser curvature most consistent with GIST.

Increased wall thickness, up to 15 mm, due to tumoral infiltration is present at right posterolateral aspect of urinary bladder that infiltrates right UVJ. Perivesical fat stranding and a few small parailiac lymphadenopathies with SAD less than 8 mm are noted. 

The prostate gland is enlarged. 

Case Discussion

Urinary bladder mass; pathology proven papillary urothelial cancer with involvement of right UVJ causing right sided hydroureteronephrosis and small parailiac lymphadenopathies.
