Idiopathic pulmonary fibrosis with unilateral lung transplant

Diagnosis almost certain

Presentation

Chest pain.

Patient Data

Age: 70 years
Gender: Male

Right-sided volume loss with mediastinal shift and diaphragmatic elevation. Reticular opacity throughout the right lung. 

Left lung is hyperinflated but clear.

Sternotomy wires. 

Case Discussion

This patient had a history of idiopathic pulmonary fibrosis with a left lung transplant, which explains the chest x-ray nicely.
