Adenoid cystic carcinoma - submandibular gland

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Painless swelling of the left submandibular region.

Patient Data

Age: 40 years
Gender: Female

There is an ill-defined soft tissue mass of the left submandibular space probably arising from the anterior aspect of the submandibular gland (measuring 35 x 30 x 25 mm). It elicits a low signal on T1, high signal on T2 with areas of restricted diffusion and vivid enhancement on post-contrast sequences. It shows pre-and retromandibular extension with the displacement of the platysma, mylohyoid muscle, and infiltration of the masseter muscle.

No cortical destruction or erosion of the left mandible.

No cervical lymphadenopathy is seen.

Case Discussion

MRI features of a left submandibular enhancing soft tissue mass probably arising from the anterior aspect of the submandibular gland extending to the pre-and retromandibular spaces.

The patient underwent surgical resection of the tumor and the submandibular gland with a histopathological exam that confirms the diagnosis of an adenoid cystic carcinoma of the submandibular gland.
