Patellofemoral maltracking

Quadriceps and patellar tendons are intact. High riding patella (Insall-Salvati ratio 1.51) with lateral translation and tilt. TT-TG is borderline (13 mm). Trochlea is well formed. Full-thickness chondral wear of the lateral femoral trochlea characterized by subchondral edema. Pre-femoral fat pad and superolateral Hoffa's fat pad edema.
Case Discussion
MRI findings of patellofemoral maltracking/instability with pre-femoral fat pad and Hoffa fat pad edema and patella alta. The diagnosis of patellofemoral maltracking ultimately hinges on clinical assessment.
