Soft tissue hemangioma

Presentation

Pain and swelling left hand (palmar surface) one month. No history of discharge, fever or trauma.

Patient Data

Age: 55 years
Gender: Male

A fusiform T1 isointense and T2 hyperintense soft tissue mass lesion is seen in the webspace between 4th and 5th metacarpophalangeal articulations. Tubular low signal intensities (blood vessels?) are seen in its center. The periphery of the lesion shows intense enhancement on the post-contrast study. No phlebolith/calcification is seen in it. No invasion is seen into the nearby structures.

Small enhancing lesion in the left hand, which is likely a soft tissue hemangioma; another possible differential can be a peripheral nerve sheath tumor (neurofibroma or schwannoma), which is, however, less likely.

Case Discussion

Procedure: Excisional biopsy.

Diagnosis: Cavernous hemangioma. Negative for malignancy.
