Metastatic small cell lung cancer

Presentation

Work up for cough and hemoptysis.

Patient Data

Age: 65 years
Gender: Male

A 195× 75× 95 mm large hetero-enhancing mass at the left upper lobe infiltrates the anterolateral aspect of the left chest wall. It extends within the left pulmonary vein causing tumor thrombosis. Additionally, left hilar structures are encased by the mass.

Interlobular septal thickening at both lungs is suggestive of lymphangitic carcinomatosis accompanied by several scattered nodules less than 18 mm, inferring metastases.

Large masses with heterogeneous enhancement at both adrenal glands are consistent with bilateral adrenal metastases.

A few small cysts are seen at the liver less than 8 mm.

The prostate gland is enlarged.

Case Discussion

Large left lung mass, pathology-proven small cell lung cancer (SCLC) with left pulmonary vein extension, lung and bilateral adrenal metastases.

Small cell lung cancers are neuroendocrine tumors of the lung that rapidly grow, are highly malignant, widely metastasize, and, despite showing an initial response to chemotherapy and radiotherapy, have a poor prognosis and are usually unresectable.
