Presentation

Non-neutropenic fevers. Background of multiple myeloma.

Patient Data

Age: 80 years
Gender: Female

Multiple rounded hypoattenuating foci within the spleen (absent on previous abdominal CT performed a fortnight prior). Given that they are new, these could represent underlying infection, with fungal infection to be considered if patient is immunocompromised.

Spleen span measures 85 mm. Multiple 'target' lesions measuring up to 11 mm in diameter. 

Case Discussion

The splenic lesions are likely to be caused by Candida microabscesses. Other fungal infections or neoplastic disease could also cause this appearance.
