Pneumatosis intestinalis with zoomed images of the abdomen and left flank. The bowel-wall gas is most clearly seen in the left flank although there is a soap-bubble appearance in the right flank also.

Case Discussion

Appearances are of necrotizing enterocolitis with pneumatosis intestinalis. There is no evidence of portal venous gas or pneumoperitoneum. Confirmation or exclusion of pneumoperitoneum is important because it turns management from medical to surgical.
