Diffuse bilateral discrete calcific foci along the testicular parenchymal bilaterally, in keeping with testicular microlithiasis.
The scrotal wall shows multiple hyperechoic cutaneous nodules with posterior acoustic shadowing, suggestive of idiopathic scrotal calcinosis.
Case Discussion
Testicular microlithiasis is the bilateral deposition of multiple tiny non-shadowing calcifications in the testes, more than five in the scan field. It is usually an asymptomatic condition and found incidentally. The European Society of Urogenital Radiology (ESUR) advises annual ultrasound follow-up until the age of 55 years if it is coupled with other risk factors for testicular cancer as maldescended testis, testicular atrophy, history of germ cell tumor and orchidopexy 1.
Idiopathic scrotal calcinosis is a rare condition where multiple nodules are seen at the scrotal wall with hyperechoic shadowing content with the presence of calcific foci. The diagnosis can be confirmed by the presence of calcifications on radiography. It is usually asymptomatic, surgery may be required for cosmetic purposes 2.
