Presentation

Know and operated case of right breast cancer. Work up for abdominal pain and chronic constipation.

Patient Data

Age: 65 years
Gender: Female

Post-operative changes are seen due to left mastectomy and axillary lymphadenectomy.

A 36 mm mass is present at the superior segment of the right lower lobe. In addition, several nodules less than 12 mm are scattered at both lung fields inferring metastases.

Asymmetrical increased wall thickness due to tumoral infiltration is present at the ascending colon, measuring about 10 cm in length. Surrounding fat stranding and a few regional enlarged lymph nodes are also observed.

The hepatic attenuation value is less than that of the spleen, suggesting fatty liver.

A few small calcified foci are seen at liver parenchyma, most consistent with healed granuloma.

A few subcentimeter simple cortical cysts are seen in the kidneys.

A 26 mm nodule is present at the right adrenal gland. Its mean attenuation values on PVP and delayed images measured 68 HU and 62 HU, respectively, suggests metastasis.

Case Discussion

Ascending colon mass, pathology-proven adenocarcinoma with small regional lymphadenopathy, lung and right adrenal metastases; in a patient with treated breast cancer.
