Abdominal whirl sign (whirlpool sign) of the mesentery

Diagnosis almost certain

Presentation

Epigastric pain after a large meal.

Patient Data

Age: 50 years
Gender: Female

Whirl-like rotation of the mesentery and superior mesenteric vein around the superior mesenteric artery, with involvement of a small bowel loop consistent with intestinal malrotation. Minimal ascites in the pelvis. Status post cholecystectomy.

Case Discussion

Example of the abdominal "whirl sign" or "whirlpool sign", a swirling appearance of the mesentery and superior mesenteric vein around the superior mesenteric artery. It commonly occurs in cases of intestinal malrotation and/or midgut volvulus 1. This patient already had a surgically treated small bowel ileus 8 years ago. This time she was treated conservatively and her symptoms subsided after a few days.
