There is elongation of the anterior tubercle of the left transverse process of C5 forming an accessory articulation with the C4 transverse process. This can be better appreciated in the sagittal and coronal reformats.
Unusual variants are not only interesting to spot, but can also have an impact in patient's management (and at the very least makes the radiologist look good).
