Accessory hemiazygos vein variant

The hemiazygos vein is seen to the left of the aorta extending beyond T8 with no midline cross or continuity with the azygos vein. The hemiazygos vein joins the accessory (superior) hemiazygos vein forming a single trunk draining directly into the left brachiocephalic vein.
Right upper lung azygos lobe and fissure containing the azygos vein that is seen normally draining into the superior vena cava.
Case Discussion
Two variations of the accessory hemiazygos vein are seen in this case:
- continuation of the hemiazygos with the accessory hemiazygos vein forming one trunk
- direct drainage of the accessory hemiazygos vein into the left brachiocephalic vein
