Presentation

Left ankle pain and swelling.

Patient Data

Age: 20 years
Gender: Male
mri

Accessory soleus muscle, which is visible between the FHL and the Achilles tendon, obliterates the Kager fat pad, with tendinous insertion into the posteromedial aspect of the calcaneus.

The joint structures, including the ligaments, are unremarkable.

Type II accessory navicular bone also is seen.

Case Discussion

The accessory soleus muscle is an uncommon anatomical variant. It may be asymptomatic or associated with pain or soft-tissue mass sensation. In the presented case it was associated with pain and swelling.
