Acetabular dysplasia with secondary osteoarthritis

Presentation

Right hip pain.

Patient Data

Age: 30 years
Gender: Female

Right worse than left acetabular dysplasia with femoral head undercoverage. Right superior joint space narrowing with subchondral lucency in the femoral head. 

Asphericity of the femoral head-neck junction anterosuperiorly with focal depression of the anterior femoral head with subcortical cysts. Dysplastic acetabulum with femoral head undercoverage. Near complete loss of superior joint space with superior femoral head chondral high signal with subchondral cysts and edema. Acetabular rim edema and an anterosuperior to posterosuperior labral tear with intralabral and paralabral cysts. Small joint effusion.

Partial tear of the anterior gluteus medius tendon. No trochanteric bursal effusion. 

The ovaries have a multifollicular, possibly polycystic appearance. Small volume of pelvic free fluid is physiological.

Case Discussion
