Dyspnea, elevated CRP and WBC. Pneumonia?

Age: 70 years
Gender: Female
x-ray

An air filled, walled structure is visible adjacent to the right heart border and mediastinum which corresponds to the distended esophagus. Achalasia confirmed by prior imaging. No features suggestive of pneumonia.

Advanced achalasia which seemingly follows the right heart border, making the appearance more confusing.
