Presentation

Posterior ankle pain.

Patient Data

Age: 25 years
Gender: Male

The Achilles tendon is surrounded by edema signal, especially along the posterior subcutaneous fat and anteriorly in the fat tissue in Kager's triangle. Mild retrocalcaneal bursitis. The Achilles tendon itself is not thickened and shows only minor signs of tendinosis.

Case Discussion

Paratendinitis is an inflammatory condition that arises in disorders like seronegative arthritis, infection, and repetitive/overuse injuries.

Peritendinitis can accompany tendon degeneration, but can also occur without underlying tendon pathology.

On MRI, peritendinitis demonstrates hyperintense edema and fluid signal on STIR weighted images surrounding or partially surrounding the Achilles tendon
