Achilles tendon tear

Presentation

Injury 1 year ago.

Ultrasound

Asymptomatic tendon - intact.

Symptomatic tendon - complete tear. Gap between two edges more than 30 mm.

X-ray

Follow the soft tissue shadow of Achilles' tendon. The one which is smooth in margin , thin is of Asymptomatic tendon.

The other shows focal thickening and weary contour due to torn tendon with retracted edge. Fat pad is not at all hazy, because of absence of fluid ( due to chronic injury - 1 yr back ).

Case Discussion

Both radiograph and ultrasound findings are classical of Achilles' tendon tear.

Comparision to asymptomatic side, helps; at least when we are learning or have doubts.

Doing an extended field to view, is be difficult initially and may also seem pointless. However, it explains pathology in much better way to others and to yourself also.
