Achilles tendon xanthoma

Presentation

Left ankle pain since months, the patient had a family history of hypercholesterolemia.

Patient Data

Age: 30 years
Gender: Male
MRI

The achilles tendon shows marked diffuse thickening with loss of concavity of its anterior margin.

Abnormal high intratendinous signal alternation at its mid and distal portions.

 

X-ray

Abnormal thickening of the achilles tendon with no evidence of abnormal calcific foci. 

Case Discussion

Considering the patient's family history and radiological features; achilles tendon xanthoma is most likely, other differentials are drug-induced tendinopathy, chronic tendon degeneration, tophaceous gout.
