ACL avulsion fracture

Presentation

Road traffic accident.

Patient Data

Age: 16 years
Gender: Male
mri

The MRI sequences demonstrate:

  • avulsion-fracture involving the majority of the tibial eminence at the tibial insertion of the ACL with complete separation of the bony fragments.
  • swollen with edema of the ACL attached to the bone fragment

  • bone marrow edema of the tibial epiphysis

  • osteochondral fracture with bone marrow edema of the lateral femoral condyle
  • moderate knee joint effusion
  • lateral displacement of the patella with patella alta (Modified Insall-Salvati ratio = 2.25) with small size of the medial facet and concave aspect of the lateral facet (Wiberg type 2 or b). 
  • both menisci, collateral ligaments as well as the medial and lateral patellar retinaculum are intact

Case Discussion

MRI features of an ACL avulsion-fracture type 3b according to Meyers and McKeevers classification with associated osteochondral fracture of the lateral femoral condyle.
