ACL avulsion fracture

The MRI sequences demonstrate:
- avulsion-fracture involving the majority of the tibial eminence at the tibial insertion of the ACL with complete separation of the bony fragments.
swollen with edema of the ACL attached to the bone fragment
bone marrow edema of the tibial epiphysis
- osteochondral fracture with bone marrow edema of the lateral femoral condyle
- moderate knee joint effusion
- lateral displacement of the patella with patella alta (Modified Insall-Salvati ratio = 2.25) with small size of the medial facet and concave aspect of the lateral facet (Wiberg type 2 or b).
- both menisci, collateral ligaments as well as the medial and lateral patellar retinaculum are intact

MRI features of an ACL avulsion-fracture type 3b according to Meyers and McKeevers classification with associated osteochondral fracture of the lateral femoral condyle.
