Acoustic neuroma with intracanalicular extension

Presentation

Right hearing loss.

Patient Data

Age: 55 years
Gender: Female

Well-defined extra-axial ovoid mass at the right cerebellopontine angle with intracanalicular extension best seen on FIESTA and postcontrast coronal T1 fat sat. It displays a low signal on T1, an inhomogeneous high signal on T2 and FLAIR with heterogeneous enhancement on the postcontrast sequences.

A mass effect is noted on the right middle cerebellar peduncle, brainstem, ipsilateral cisternal portion of the trigeminal nerve as well as the 4th ventricle which is laminated and displaced to the left with no significant hydrocephalus.

Case Discussion

MRI features most consistent with acoustic neuroma of right CPA with intracanalicular extension, compressing the middle cerebellar peduncle, brainstem, cisternal portion of the trigeminal nerve, as well as the 4th ventricle with significant hydrocephalus.
