Acoustic schwannoma

Diagnosis almost certain

Presentation

Recurrent dizziness. Falls following sudden left side hearing 8 months ago. Clear ENT exam. Significant left side hearing asymmetry. MRI to rule out IAM/CPA or intracranial abnormalities.

Patient Data

Age: 80 years
Gender: Female

1.8 cm left cerebellopontine mass with intracannilicular extension.

Background moderate small vessel ischemic disease.

Case Discussion

Large numbers of MRIs of the internal auditory meatus (IAMs) are undertaken, in large part to assess for vestibular schwannoma. They can be at the cerebellopontine angle, purely intracanalicular, or as in this case involve both.

Most centers now do a T2 3D sequence through the internal auditory meati which exquisitely illustrates the vestibulocochlear nerves.
