Acoustic schwannoma - intracanalicular

Presentation

Mild right-sided sensioneural hearing loss and tinnitus.

Patient Data

Age: 50 years
Gender: Female

MRI demonstrates a right entirely intracanalicular enhancing mass. Minor expansion of the canal. No cerebellopontine angle component to the lesion. It is seen attached to the right 8th nerve.

Case Discussion

This patient was referred to us with a history of tinnitus and hearing loss on the right side. Based on the history and MRI findings, a VIII nerve schwannoma is the first diagnosis.

High resolution thin T2 sequence (e.g. FIESTA) is needed to detect small intracanalicular acoustic neuromas which may be missed on conventional images.
