Acromial fracture

Presentation

History of fall from bike 2 months back. Injury to left upper chest. History of left upper posterior rib fracture. Complaining of pain in left shoulder. Referred to check rotator cuff integrity.

Patient Data

Age: About 65 yrs
Gender: Male
Modality: Ultrasound

Intact and normal echopattern of rotator cuff.

No bursal effusion.

No gleno-humeral effusion.

Acromion shows cortical breach, fragmentation.

Ultrasound findings favoured acromian fracture or very less likely an os acromiale. Shoulder axial view radiograph was done to differenciate them.

Modality: X-ray

Fracture line through acromion process of scapula.

If corticated margins are noted on both sided of lucency, it favours os acromiale.

Findings of normal and intact cuff with acromial fracture.
