Presentation

A fall from bike two months back. Injury to the left upper chest. History of left upper posterior rib fracture. Complaining of pain in the left shoulder. Referred to check rotator cuff integrity.

Patient Data

Age: 65 years
Gender: Male
ultrasound

Intact and normal echo pattern of the rotator cuff. No bursal effusion. No glenohumeral joint or subacromial-subdeltoid bursal effusion. Scapular acromion process cortical breach.

US findings favored ...

x-ray

US findings favored acromial # over less likely os acromiale. Shoulder axial view XR was ordered to differentiate them.

Fracture line through the acromion process of the scapula.

If corticated margins are noted on both sides of lucency, it favors os acromiale.
