Presentation

Pain in the right shoulder after fall down.

Patient Data

Age: 75 years
Gender: Male

Disruption of the AC joint and clavicle displaced posterior into the trapezius

AC ligament: ruptured

CC ligament: ruptured

Joint capsule: ruptured

Deltoid muscle: minimally detached

Trapezius muscle: detached

Coracoclavicular distance: 26 mm.

Case Discussion

Posterior dislocation of the distal clavicle with respect to the acromion, with the clavicle protruding into the trapezius muscle, is a type IV ACJ injury.
