Acromioclavicular joint injury - type II

The distal clavicle is elevated beyond the acromion with widening of the joint space. The coracoclavicular distance is not significantly increased and therefore, this represents a type II injury.
Case Discussion
Acromioclavicular injuries are common and can easily be overlooked. Ensure that the inferior surface of the clavicle and acromion are aligned. If they are not, think ACJ injury and use the Rockwood classification. If alignment is normal, look carefully at the joint space - type I injuries can be subtle and even occult on a plain x-ray.
