Presentation
Acute right upper quadrant pain with anorexia and nausea.
Patient Data

There is oedematous/thickened wall of the gallbladder (measuring up to 6.0 mm in maximum size unilaterally), however no CT detectable radiodense calculus or mass lesion is appreciated.
Mild amount of pericholecystic free fluid is noted.
Diffuse fatty changes of the hepatic parenchyma is seen as well.
Case Discussion
Abdominal ultrasound also showed gall bladder wall thickening with pericholecystic free fluid (not shown).
The absence of radiodense calculus, no existence of definite other pathology, and clinical presentation of the patient with current CT findings are more in favour of acute acalculous cholecystitis and diffuse hepatic fatty changes as described.
