Presentation
Shortness of breath, fever, general weakness and palpitation.
Patient Data
Hyperdynamic left ventricle, mildly dilated (LVIDd = 61 mm), stroke volume = 50 mL.
Large vegetation on the ventricle side of the aortic cusp (30 × 15 mm).
Severe aortic regurgitation.
Dense continuous-wave Doppler signal with a steep slope (pressure half time <150 ms) indicating severe aortic regurgitation.
Moderate mitral regurgitation.
Case Discussion
This patient presented to the Emergency Department with dyspnoea and fever that started a week prior.
Clinical examination revealed bibasilar fine crackles.
Blood culture was obtained immediately, which was negative as the patient had a prior treatment of antibiotics.
After an immediate admission to the intensive care unit, the patient was stabilised and transferred to emergent cardiac surgery.
