Distended vermiform appendix measuring up to 19 mm in width, containing fluid and gas. Some gas locules appear to be intramural. There is wall enhancement except at the tip, but minimal wall thickening. The surrounding fat planes are heterogeneous. Findings are consistent with acute appendicitis.
Case Discussion
Obstruction leads to distension and ischemia. Intramural gas suggest necrosis.
The surgeons noted a perforation.
