Acute appendicitis

There is a long pelvic fluid-filled appendix (11 cm) with a thick enhanced wall (2 cm), containing an appendicolith (1.8 cm) obstructing the ostium with no evidence of wall defect. Periappendiceal fat stranding with thickening of the peritoneal reflections and free intraperitoneal fluid in the pelvis and ileus.

CT features of a long pelvic acute appendicitis. The pelvic location is seen in around 30% of cases.
