Marrow edema at both articulating surfaces of the right sacroiliac joint with post contrast enhancement. No significant effusion is seen. Normal joint space. No evidence of sclerosis or any chronic changes. There is also associated inflamation of the right iliacus and gluteal muscles manifested as abnormal bright T2 signal and post contrast enhancement enhancment.
Evident sacroiliitis highly suggested by the bone marrow edema and the corresponding post contrast enhancement. No radiological signs of chronic changes of the sacroiliac joint together with the acute presentation of the patient and the positive lab results for Brucellosis suggest the diagnosis of acute Brucella sacroiliitis.
