Acute calcular cholecystitis and abdominal wall lipoma

Diagnosis almost certain

Presentation

Right upper quadrant pain with tenderness.

Patient Data

Age: 35 years
Gender: Female

Distended gallbladder with thickened wall, and mucosal enhancement, containing numerous gallstones isodense to bile but visible on ultrasound (not shown) with an impacted stone within the gallbladder neck. Intramural diverticulae (Rokitansky-Aschoff sinuses) are noted within the gallbladder wall (adenomyomatosis). Pericholecystic inflammatory fat stranding is noted.

A well-encapsulated mass of fatty density is noted within the right abdominal wall of intermuscular location between the external and internal oblique muscles.
