Right upper quadrant pain.

Patient Data

Age: 65 years
Gender: Female

The gallbladder is distended up to 58 mm, accompanied by wall thickening and surrounding fat stranding. A 12 mm stone is noted in the gallbladder neck.

Case Discussion

Features on CT scan are typical for acute cholecystitis due to the gallbladder neck stone.

Main CT findings include :

  • cholelithiasis: gallstones isodense to bile will be missed on CT
  • gallbladder distension
  • gallbladder wall thickening
  • mural or mucosal hyperenhancement
  • pericholecystic fluid and inflammatory fat stranding
