Presentation

Painful sudden swelling and anterior bulge of right upper eyelid outer part.

Age: 25 years

There are remarkably enlarged palpebral and orbital parts of the right lacrimal gland encasing and mildly indenting the adjacent eye uveo-scleral layer with remarkable right upper eyelid and preseptal soft tissue swelling and anterior bulge more prominent in the temporal side. Mild right sided proptosis is also seen more appreciable on sagittal images. 

Case Discussion

The case is a 25-year-old female with a history of painful and sudden swelling of the right upper eyelid more on the outer side. The non-contrast orbital MDCT requested a remarkable enlarged right-side lacrimal gland palpebral and orbital segments that were lacrimal gland palpebral and orbital segments detected compatible with dacryoadenitis. Dacryoadenitis could be infective or non-infective and in cases of non-infective, IG4 related disorders, sarcoidosis, granulomatosis polyangiitis, Sjogren's, or non-specific orbital inflammation can be considered in the differential diagnosis.
