Acute disseminating encephalomyelitis (ADEM) with bilateral optic neuritis

Diagnosis almost certain

Presentation

Loss of vision in both eyes following fever.

Patient Data

Age: 5 years
Gender: Female

Asymmetrical patchy white matter with involvement of overlying cortex at places in bilateral frontoparietal lobes, also extending to body of corpus callosum on right side.

Hyperintensity / edema is seen involving intraorbital, intracanalicular, intracranial segments of both optic nerves, optic chiasma and anterior aspects of bilateral optic tracts suggesting bilateral optic neuritis.

Case Discussion

In the present clinical context, above findings suggest likelihood of Acute disseminating encephalomyelitis (ADEM) with bilateral optic neuritis.
