Acute hemorrhagic encephalomyelitis

Presentation

Seizures with encephalopathy, on ventilator

Patient Data

Age: 5 years
Gender: Male

Edema and hyperintensity on T2W/FLAIR are seen involving bilateral cerebral cortex, white matter, basal ganglia and corpus callosum with areas of diffusion restriction and foci of hemorrhage. Subtle hyperintensities on T2W/FLAI are seen in bilateral thalami. Foci of diffusion restriction are also seen in bilateral cerebellar hemispheres.

Edema/hyperintensity is seen along intracranial segments of both optic nerves, optic chiasma and possibly along optic tracts.

Case Discussion

Above findings suggest likelihood of acute hemorrhagic encephalomyelitis (AHEM) or Hurst disease. The patient was found to have an ongoing measles infection.
