Mild high FLAIR and high DWI signal (without low ADC signal) in the lateral putamen, external capsule, and claustrum and insula on both sides, as well as pons and mesencephalon slightly high signal.
Case Discussion
Symmetrical high FLAIR signal, affecting the lateral putamen, the external capsule, and claustrum on both sides as well as the pons and mesencephalon, primarily suspicious of acute hyperammonemic encephalopathy in acute liver failure, but without evidence of chronic hepatic leukoencephalopathy.
