Presentation

Acute hemiplegia in the emergency department.

Patient Data

Age: 50 years
Gender: Female

The diffusion-weighted images show an area of restricted diffusion that involves the right internal capsule posterior limb and genu with bright signal at (DWI) and dark signal at (ADC) map.

Case Discussion

The case shows restricted diffusion which is a classic sign of acute brain infarct. It does not show the classic features of anterior choroidal artery syndrome (rare entity characterized by the triad of hemiplegia, hemianaesthesia and contralateral hemianopia secondary to infarct of the anterior choroidal artery). The internal capsule is a deep subcortical concentration of white matter fibers.
